The forms on this page must be filled out before each appointment; it is especially important to verify your medications. Please either bring these pages to your appointment or email them to us by clicking the send email button at the top of the page.

This is critical for your well-being, as each clinician must review these documents thoroughly before they see you and can begin to treat you. As such, if they don’t have this at the start of your appointment it has to be rescheduled.

New Patients

Please call our clinic at (248) 335-7740 for an appointment, we have to input you into our HIPAA-compliant system before you can make an appointment online.

Returning Patients

To schedule your next appointment please call our clinic at (248) 335-7740 or log in to your patient portal. If you don’t see any available appointments online, please give us a call and we will get you on the schedule. 

*Policy Update* Late Cancelations &
No-Show Appointments

We have made some policy updates to keep our office and your experience running smoothly. Please show up a few minutes prior to your appointment with your forms filled out. If you need to reschedule, please give us 48 hours’ notice. Please note that weekend hours are not included in this timeframe, as our office is closed. To maximize our efficiency in seeing patients, serving patients by appointment is a requirement, and we are not accepting cash as a payment method after August 31st. No-shows and late cancellations can have a significant impact on our practice beyond just financial implications. No-shows and late cancellations cause delays in healthcare delivery to other patients, some of whom are dealing with serious medical issues and thus may incur a fee. If there are consistent no-shows and late cancellations, you may be subject to discharge just so we can continue to provide the standard of care you have grown to expect from us.

At Grunberger Diabetes & Endocrinology we respect your time. Your appointment time is reserved solely for your care. Expect to sit in the lobby briefly with your completed forms before seeing your clinician or another specialist.
