The forms on this page must be filled out before each appointment; it is especially important to verify your medications. Please either bring these pages to your appointment or email them to us by clicking the send email button at the top of the page.

This is critical for your well-being, as each doctor must review these documents thoroughly before they see you and can begin to treat you. As such, if they don’t have this at the start of your appointment it has to be rescheduled.

New Patients

Please call our clinic at (248) 335-7740 for an appointment, we have to input you into our HIPAA-compliant system before you can make an appointment online.

Returning Patients

To schedule your next appointment please call our clinic at (248) 335-7740 or log in to your patient portal. If you don’t see any available appointments online, please give us a call and we will get you on the schedule.

At Grunberger Diabetes & Endocrinology we respect your time. Please show up a few minutes prior to your appointment with your forms filled out. Expect to sit in the lobby briefly before seeing the doctor or another specialist.

The appointment time is reserved solely for your care. We do not double-book appointments. If you need to reschedule, please let us know at least 24 hours in advance. We charge a nominal fee for no-shows or appointments canceled with less than 24 hours notice.
